I am covered by my own health insurance policy. In case anything happens to me on this trip requiring medical treatment in which I am unable to sign for myself I will provide a copy of pertinent information to be carried by the team leader and destroyed or returned to me upon return from the mission assignment.

I am a parent or guardian registering on behalf of a minor child. I assume responsibility for the care, supervision, and medical needs of this child while on this assignment. In the event of my inability to exercise these things, I hereby authorize the team leader or other recognized leader from VBC to act on my behalf.
